Submission Methods:

1. Come to school in person during the office hours for the collection and submission of application form
2. By post – G/F to 3/F, On Ning Road, Yuen Long, N.T.

Required Documents:

1. Original and photocopy of Birth Certificate
2. Original and photocopy of Immunization Record
3. Two recent passport-sized photos
4. Two self-addressed envelopes with stamps and children’s name
5. Application fee $40
